Passport helps adults with a developmental disability who have left school find more ways to participate in their communities. Passport:

  • eases the transition from school to life as an adult in the community,
  • encourages independence,
  • builds social, emotional and community participation skills, and
  • promotes continuing education and personal development.

Through Passport, people can receive funding for activities that help them:

  • grow personally
  • use community programs
  • develop work and volunteer skills
  • join local clubs
  • reach their goals.

Individuals and families can choose to:

  • receive Passport money directly so they can purchase their own services and supports,
  • get services and supports through community-based agencies, or
  • get services and supports through a combination of both.

Mentoring Program

Passport also has a mentoring program for students with a developmental disability who are 14 - 21 years old. Students are paired with experienced adult mentors. Students and mentors work together to decide what steps they need to take to achieve the student's goals.

Who can apply

Passport is for young adults with a developmental disability who are no longer eligible for school supports, but who would benefit from community participation supports. You cannot receive Ontario Disability Support Program Employment Supports at the same time.

You can live with family, independently or in supportive living (like a group home). But first priority goes to people who are living at home with their families.

We know that more people still need support, and we will continue to do more to improve services for people with a developmental disability.

How to apply

Complete the Passport application form if:

  • this is the first time you are applying
  • there has been a significant change in your care needs or circumstances since your last full Passport application
  • three years have passed since you last applied.
