We will send you a registration package when we receive:
- a support order,
- a support deduction order, or
- a domestic contract that has been filed in an Ontario court
The package has some forms for you to fill out and send back to us.
The forms are:
- Support Filing Form. In this form, you provide us with such information as name, address, phone number for you and the person paying the support.
- Registration for Direct Deposit. When the person paying the support sends us your support payment, we will forward the money to you by direct deposit to your bank. You fill out this form to provide us with the information we need.
- Payor Information Form. You fill out this form to give us information about the person who is paying support to you. We need this information to enforce your support order or domestic contract.
- Statement of Arrears. You fill out this form if the person paying the support owes you any support payments. With the information you provide in this form, we may begin to collect the missed support payments (called arrears).
We need complete and accurate information from you so that we can begin to enforce your support order or domestic contract and collect your support payments. This is especially important if the person paying the support does not have a regular source of income, such as an employer.
Help us enforce your support order or domestic contract.
Make sure the information we have about you and the person paying support is up-to-date.
Let us know right away if you:
- move
- change your telephone number(s)
- change your name (You will have to send us proof of your name change, such as a copy of the birth, marriage certificate, sworn affidavit or a court order.)
- have new information about the person paying support, for example, a new address, telephone number or job
- receive a support payment directly from the payor. (Once your case is registered with the Family Responsibility Office, support payments must come through us.)
Also, let us know if:
- your support order or domestic contract changes, or
- the support obligation ends.
